Diabetes Management in Assisted Living: A Personalized Approach

How Heathwood Helps with Diabetic Management

Diabetes is a chronic condition causing high blood sugar due to insufficient insulin production (Type 1) or ineffective use of insulin (Type 2). Key symptoms include extreme thirst, frequent urination, fatigue, and blurry vision. According to the World Health Organization, 828 million people worldwide suffer from diabetes as of 2024, and 2 million deaths result from it annually. While the condition is often lifelong, managing diabetes through diet, exercise, and medication can prevent severe complications.

Assisted living facilities can help residents effectively manage diabetes through meal planning, medication monitoring, and lifestyle support.
Personalized care plans and trained staff promote better outcomes and daily safety.

Key Aspects of Diabetes Management in Seniors

At Heathwood Assisted Living, we find that personalized care plans and nutritious diets make all the difference for our residents. Our staff members are trained to recognize the symptoms of high and low blood sugar and to intervene appropriately. We also promote daily physical activity like walking and light cardio to maintain stable blood sugar and overall health and wellness.

We focus on the following for safe and appropriate diabetes management in seniors:

Medical Care & Monitoring

We assist with blood glucose monitoring and medication management. While residents may self-administer some medications, insulin injections are often performed by licensed practical or registered nurses.

Dietary Support

Nutritional plans for our residents with diabetes focus on balanced, diabetes-friendly meals that are low in sugar and high in fiber, often tailored to individual needs. Regular, consistent carbohydrate intake is emphasized to prevent spikes.

Staff Training & Early Detection

We train our staff to recognize symptoms of hyperglycemia (high blood sugar) and hypoglycemia (low blood sugar), such as frequent urination, fatigue, and unexplained weight loss. Depending on individual circumstances, our staff may intervene directly or refer residents to medical staff for assistance.

Physical Activity

We work with residents to incorporate regular,
gentle physical activities—such as walking or group exercises—into daily routines to help control blood sugar.

Individualized Goals

We tailor every care plan based on the resident's overall wellness, other conditions, and cognitive status, specifically focusing on avoiding hypoglycemia.

We take special care when it comes to the diets of our residents with diabetes. We often focus on stabilizing blood sugar using the "
Diabetes Plate Method": 50% non-starchy vegetables (spinach, broccoli), 25% lean protein (chicken, fish, tofu), and 25% complex carbohydrates (brown rice, quinoa, beans).

Our diets prioritize high-fiber, low-sugar, and low-sodium foods, often following Mediterranean principles to limit processed items and saturated fats. However, we always take individual needs and preferences into account. Our goal is to work with every resident to enable them to enjoy a diet that is delicious, nutritious, and carefully calibrated to their individual needs.

Common Procedures

Diabetes is a complex condition requiring constant monitoring and management. Our staff members assist residents with the following:

  • Blood Sugar Checks: We help residents check their blood sugar throughout the day to ensure their sugars are within safe levels.
  • Routine Care: We assist and encourage our residents to get regular check-ups, including annual eye exams, to detect complications early.
  • Weight Management: Monitoring for unexplained weight loss is a key part of care, and we play a vital role in that.

Personalized Care at Every Turn

When it comes to diabetes management, we understand that no two cases are alike. We always treat the whole patient, not just a single condition. For instance, for residents with severe complications or advanced dementia, we may focus more on comfort and avoiding extreme symptoms rather than strict glucose targets.

Trust Heathwood Assisted Living to Help with Diabetes Management

When one of our residents is diagnosed with diabetes or when a new resident already managing the condition moves in, our staff springs into action to help them. We ensure residents adhere to a regular medication schedule so they receive their medications at the appropriate times and don’t risk missing doses. Our nurses can also administer insulin injections when necessary. We help residents manage diabetes through diet with three nutritious, chef-prepared meals each day served restaurant-style in our beautiful dining room. And of course, every plan is developed with the needs of the individual in mind.
